Medication management is a monthly service we provide that helps assess medication compliance and  evaluate how our client is feeling on their current medication regime. We understand the importance of medication compliance.

 

A lack of medication compliance is one of the top two reasons which can impact a senior’s ability to live independently and affect their overall health status. Our advanced practice nurse will visit you on a monthly basis to evaluate your current health status and review the medication plan.

 

TouchStone has partnered with V & T Pharmacy to provide in-home delivery of blister medication packets which can be provided to you on a weekly or monthly basis. V & T Pharmacy will assist with managing and refilling medications, coordinate with your physician to ensure no lapse in refills and is available for questions regarding medications.
